Bob Byars said that he couldn't get them any more.

I bought one from PIM last month to use as a back-up in case I needed to go that route. It is aluminum (the boot says CNC) and cost $159.50 + shipping. It does not come with a push rod.

After it arrived, I was looking at it and noticed that the bleeder screw would not seal. I discovered that the hole for the bleeder screw was not tapped deep enough. I called Jerry at PMI and he said to return it, but that he didn't have another one. I told him that I should be able to use a bottoming tap to fix it, but if that didn't work, I didn't want to be stuck with it. Jerry said that if my "fix" didn't work, just send it back.

I think that Jack DeRyke posted that Marino at Pantera East was having some long throw slaves cast in stainless steel, but I haven't confirmed that.

As I posted above, the long throw slave will increase the distance that the throw-out bearing moves, but at the cost of increased pedal effort.
