rear wheel bearing replacement

Can someone tell me or give me directions on how to replace the rear wheel bearingS ?
here whats going on I can slightly move the rear wheel in +out slightly and if I push the sides towrads the front , then rear it moves pretty good- so I assuming ( ya know what that means) that the rear bearing & probabaly races need to be replaced, before i can do a proper alignment.

So can someone tell me where I can get these bearing and is there a special tool to replace these bearings as I think I heard there was- any help much appreciated
