tie rod ends & steering rack

I was rebolting up the tie rods yesterday and when I put the new nut on the driver's side and went to tighten it up the stud was turning. Does that mean my tie rod end is defective? Are they hard to change? Are they available from the parts dealers?
2nd ?? I have redone practically the entire car from the ground up. About the only thing I haven't touched is the steering gear box. Is it difficult to get out of the car? What is involved?
