THis is a very smart system. The large grilles cover a hole you make in you make in the door panel, directly behind the armrest / doorpull. This lets air travel from the cockpit, thru the grille into the door. The small vents in are installed on the outside of the door jamb (not sure if this is the correct term) just above the door lock mechanism. THis lets the air escsape out thru to the outside.

I had this system on my car. It made a noticeable difference.
Ok, makes sense to me. I dissasemble doors all the time and they do have a vent system in most. It will also help the door close better because trapped air can escape. Most do have the rubber flaps. I don't know if this is true but years ago my friend had a 66 Barracuds Formula S. He told me that that was the first car to employ auto cabin vents. I think they were in the rear though. I don't need them for my button car because I have the mystery pull knob.
