You probably have a kink in the system somewhere.
Check the transmission end and be sure that the cable housing is tight and that the cable ends are well tightened. Also see if there are any sharp bends in the cable after it starts forward from the transmission. the cable housing should travel toward the front of the car in smooth easy curves.
After that if nothing is found take the cable out, clean it well and use some dry graphite lubrication, no oil or grease as that can go into the speedometer and mess it up. The gears could be worn although they should not be.

Do not take the little drive mechanism on the transmission that the cable screws on to off as a piece of it can drop into the transmission when you do that if you are unaware that it is there.
