First step was to seal the hole. I bought some ABS sheets from TAP and cut them to match the speaker hole and epoxied them into place:
CB6239C8-187D-4D0C-93EF-4A49A0549986
I then took some 3/32" ABS and cut it to the shape of the door:
6E5F522A-257D-4A9A-9844-F5E40466E10C
I melted these "elevator bolts" into the ABS:
E9F3F9B3-F382-4924-A67C-52F5BA2078BBI then glued a thin sheet of  3/16" foam to the plate then glued the vinyl to the foam.
My local upholstery shop had some vinyl that is an exact match.
It turned out pretty good, I think.
